Financing Your Care
We are in-network providers for both Florida Combined Life and Cigna Insurance.
We accept all PPO insurance plans and would be happy to file your insurance for you. If you are out of network, we collect our standard fees at the time of service, submit the claim on your behalf, and your insurance company will send the reimbursement directly to you. We cannot submit to secondary insurance, but we are happy to help you complete the claim form.
One of our most exciting financing options is our exclusive Cooper Creek Dental Signature Plan. It is a custom-designed savings plan for patients without insurance. The plan is offered at $250 annually per individual and $50 for each additional family member in the same household. One of the highlights of the plan is that it provides 50% off all hygiene services and 20% off most other procedures. It is an annual membership for you and your family.
Payment options:
For your convenience, we accept cash, personal checks, money orders, and major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.

Please note: Payment is due at the time of service.
As a courtesy, we will bill your insurance company and track claims on your behalf. Please keep us informed of any changes to your insurance plan. You are responsible for all fees charged by our office, regardless of your insurance coverage. Most insurance companies respond to claims within four to six weeks. Any remaining balance is your responsibility.
