Insurance

We are paticipants of the following insurances:  Florida Combined Life,  and Cigna Dental.  We will no longer be participating providers with Delta as Of July 1st.  

Should you have a different insurance, we will collect our standard fees from you and submit to your insurance company. We will gladly submit your insurance for reimbursement to you.  Any questions on this process in regards to your plan, will need to be directed to your inssurance company.  We do not submit to secondary insurance.
